I have 4 LORs from professors at my School and one from a doctor I shadowed currently. Would another LOR from a second doctor I shadowed be better than an LOR from the office manager where I volunteer at a free clinic?

Content is FAR more important then the status of who writes the letter. If the doctor you shadowed can write something great then go for it. If you have a long term commitment at your volunteer place and this person can write a good letter and has had a lot of time to work with you then go for that. Just because a dr writes you letter doesnt mean its good or it means a lot. They wont like see you have 2 doctor letters and be like "o wow this person is going to be the best damn doctor ever as 2 doctors vouch for him."
