When you are asking professors for recommendation letters, what do you have to ask them? Particularly for non-science professors who may have never written a rec letter for med schools before?

For instance, I am thinking of some logistic things like asking them to use a letterhead and maybe asking them to include your AMCAS ID# just in case? Is that necessary? Is there anything I am forgetting?

(Oh, and reminding them to refer to your legal name if you go by some other name that they know you by...)
 
Are you able to get a committee letter at your school? If so, they usually have a form to fill out and give to your professor. The form usually outlines exactly how they should construct the letter, and then the committee takes care of the rest. If your profs need to submit directly to AMCAS then there is a form that needs to be printed out from AMCAS with your AMCAS ID and a bar code I believe. Someone else may answer that better than I can. However, if you have a committee at your school then use it. So much easier than trying to coordinate everything yourself.
