LOR submission question

I have a few questions regarding getting a LOR from my dentist:

1) How is the overall structure of the LOR which my dentist will be writing? I understand that when I add the email in the AADSAS application, my recommenders will have direct access to the electronic version of the LOR. My question is, how is this electronic LOR structured? Is it comprised of a series of objective questions and then a blank space where my recommender can write freely about me? How will my recommeder be able to add the letterhead when submitting electronically?

2) How difficult is it to access the electronic version of the LOR, fill it out, and submit it successfully? Does it involve a lot steps such as downloading, uploading, etc. Seems like a weird question, but the reason I'm asking this is because the dentist I've shadowed is quite old (about to retire in a few years) and judging from how he used the computer to check his emails and browse the web, I don't think he is THAT tech-savvy. Thus, I'm just worried that he might run into a few problems while submitting the LOR.

3) The dentist I've shadowed initially earned his Bachelor of Dental Surgery from outside the US, but later got his license to practice in the US and has been practicing here for over 30 years. Will dental schools look at the LOR written by this dentist unfavorably by any chance because he got his BDS from outside the US....?

1. They receive an e-mail that gives them access to a portal in which they upload a digital copy (Word, PDF, etc) of their letter. There's no template for their letter. They upload what they've already written. LOR's typically address an applicant's personality, school/work ethic, strengths, accomplishments, and opinions on why he/she is a good candidate for dental school.

2. The e-mail they receive provides all the instructions. If your letter writer is smart enough to read and write a letter, surely he can follow simple upload instructions. It's pretty straight-forward.

3. Doubtfully. A legally licensed dentist is a legally licensed dentist.
