Hi...Does anyone know if you should include your grades and personal statement when asking for an LOR or if your CV is enough

Ask!

One of my LOR writers asked for a CV, transcript, and board scores. The other two only requested a CV and when asked, "do you want my transcript?" They replied "no." So the easy solution to guessing what the other person is thinking is to ask.
 
Asking is good.

This is what I give: (in a folder with pockets)
One side: front to back: 1. typed intro thing thanking attending for the letter and detailing what is in the folder. 2. rec letter cover sheet from my school saying where the letter should go and saying that I have waived my right to look at the letter 3. CV 4. PS
Other side: front to back: 1. stamped envelope addressed to my student affairs office 2. research examples: manuscripts, abstracts.
