LORs for SMPs

Which, if not all, SMP's ask for letters of rec? I didn't have the work ethic in college to get the grades(3.15 GPA), so what makes the SMP admissions people think that I had the initiative to go to office hours? I can get one from an employer, but I graduated a few years ago and really don't think I made a lasting impression on any professors. Also, if LORs are absolutely necessary how do I go about asking professors for them, and do I have to mention if I'm applying for an SMP and not med school directly? Or can I recycle them (IE apply for med school in June, and if I don't get in use the same LORs to apply to an SMP a few months later)
 
I think all the SMPs ask for letters of recommendation. I asked my professors to write a slightly different verision for the SMP after I didn't get into medical school, it was all kinds of embarassing. I was one science professor short when I applied, so I just asked a teacher who seemed nice whose class I had gotten an A in. My premedical advisor tells me it was a very bland letter, I told I figured it would be.
