More LOR questions! Please clarify this without biting my head of please...
1.) To my understanding, do you print out a LOR matching form for each person giving a recommendation and staple them to the recommendations or do I fill out the matching form online?
2.) Do I have to print out a LOR matching form and send it with the recommendations to or can the people giving recommendations send them directly to this address:

Print out the matching form online. Fill it out. Staple it to the Letters of Recommendation. Put it in an envelope, and send it to AADSAS.

You can give the form to your professors and have them submitted within the envelope (with your waive my right checked off). Whichever is easier for you.
