I need advice: should I even apply?
51 EPC after taking it 3x
top 25%
5 externships with letters from 2 PDs, 2 omfs chairs and private practice owner.

I know we always say to apply and just do a noncat but this is getting expensive!
 
How did you prep for CBSE? Did you do all of UW and go through pathoma n sketchy? BnB? o Bootcamp, How did you prepare each time you took the exam and what did you try changing?

the first time i wasnt able to make it all the way through UW and mainly used bootcamp but these last two attempts I focused on UW and pattern recognition. I feel like I know the material, but when the exam comes I fumble.

this is your issue. after 51 x3, you should understand that you don't know the material. Knowing material and "pattern recognition" are pretty opposite things.

You have to highlight your weak points and go back to didactic resources (BNB, pathoma etc) to review these from the bottom up. Should take you like 3-4 weeks. Then return to Qbanks and be sure to include as many CBSSAs as you can.

Do a non-cat and retake the CBSE, shoot for a 60-65+ depending if you wanna do 4 vs 6 year
 
Careful with doing a non-cat with a less than ideal score. You can fall into a difficult cycle of doing multiple non-cats. I'd just focus on getting that score higher, even if it means a gap year. That extra year means nothing in the grand scheme of things.

I think in today's application atmosphere, the 4 year programs have gotten very competitive (high CBSEs, or medium CBSE + noncat). The 6 year programs have fallen out of favor but are still very self selective (you need a good CBSE, bc they can't risk a failed step). What that means is, the people who think "I'll grind with multiple noncats and a low CBSE", that's a tough road bc the 4 year programs don't go for that anymore, and neither do the 6 years. So just be careful before starting a noncat
 
If you don't match, DO NOT do a non-cat.

you wont improve on your 52.

take a chill private practice job, or a community health clinic, and use all that time to study like mad for the CBSE, retake it in july.
