I had this case, 30-year-old girl comes into the ER With a one day history of burning on urination and painful lymph nodes in the inguinal region Bilaterally. No fever no white count just hematuria and pyuria on UA. Can UTI cause lymphadenopathy in the groin like that? She had two mobile firm tender lymph nodes in each inguinal canal. She had no vaginal complaints of discharge or dyspareunia. Has anyone seen this?

Common things being common I would consider Herpes.
 
I would consider LGV as well. It's probably more likely that a patient has concurrent LGV and UTI than that she has lymphadenopathy from a UTI.
 
Also, when you consider that the bladder & ureters' lymphatic drainage is the external iliac, obturator, internal iliac (hypogastric), and common iliac nodes, it seems even less likely that a UTI would cause inguinal lymphadenopathy.
 
Glad we had this talk. Saw a thirty something pt today with b/l swollen , tender lymph nodes, just out of prison with a history of LGV. Cultured, treated and rendered to local STD clinic. So I've seen a possible 2 cases within the same month, my attending said he had t even thought of that diagnosis in at least 15 years. I guess it's becoming more common. Thnks everyone for your input, this site has been a great learning adjunct and I appreciate the vast wealth of knowledge that everyone shares. Cheers
