Is the cigarette even important to the point of the story?

Another way to look at it: They don't show characters on TV getting dressed every morning even if it is the most important day of their life.
 
If you don't mention the cigarette does the story change in any meaningful way? To a reader, most likely not, and it'll get rid of the chance of someone having an adverse reaction to the story.

The only way I can imagine the cigarette being important is if it were mentioned repeatedly with some symbolic significance, which would romanticize the smoking and be even worse.

Definitely not worth the risk of mentioning it.

you should end it with "his final request was to smoke a cigarette with me" and leave it at that, then maybe you'll get some curious adcoms who will have to interview you to find out if you smoked with him or not 😉
 
Fond memory of mine, and the last memory i shared with someone was smoking a cigarette with them. I became a friend of the patient. He passed away as he was at the end of his life when he asked to smoke with him before i leave as a farewell. When i left, he passed away.

I wanted to mention this in my essay as it impacted my life. Everyone told me this will raise red flags and will question my "morals." Is this a nono?

Mentioning smoking is definitely an unnecessary risk. Almost as bad as the risk from actually smoking. You should quit, if you haven't already.
