Can't speak about SUNY Downstate and Einstein, but none of the others should be considered "malignant". I found some of the residents at Columbia to be more abbrasive than at other institutions, but that's all. I interviewed at all these places and questioned the residents extensively looking for signs of malignancy, and they're all good programs from that point of view.
