sometimes a program will release you from the NRMP contract if you have another position arranged
I would like to know if you did match and went to the program, is it possible to switch programs the following year? For example: IM PGY-1 then change to Anes PGY-2 the following year, if it can be done please explain. Any response would be appreciated
Just a hunch, but I think that programs only sign you to one year contracts at a time, so I guess you're not bound to stay at the same residency for the whole duration. that's just a hunch, sorry I couldn't help anymore.

To leave one program for another though, usually requires a "letter of recommendation" from the previous program director. You have to leave in good faith and be "in good standing," whatever the heck that means. I don't know that this is a universal requirement, but we've had some residents leave our institution mid residency, and it was really quite awkward.

The above posters are correct: when you apply for the NRMP you sign a contract which states that you will accept any position offered to you.
Will the NRMP punish you for not doing so? They do by not allowing you to accept another position for a year.Are there ways around this? Sure. It happens every year.
But the biggest difficulty will be between the programs. As another has stated, you must have a letter of approval for breaking your contract from the matched program and the other must be willing to accept you - some are unwilling to do so for fear of being seen as "stealing" residents. You are treading difficult waters - if you are unable to accept another offer, your program may make it unpleasant for you while there.
