Sorry in advance about what may be a stupid match question. I was briefly looking at some of the match data that just came out and it seemed that traditionally competitive residencies like ENT had some unfilled spots. Lets say that a student applies to every single possible ENT program in the country...does that mean he would have a good chance of matching even if his stats are not as stellar as others seeking ENT, albeit more picky, candidates?
 
the one word answer is no

not everyone who applies to a program will get an interview. Not everyone who interview at the program will be ranked by the program. If you are not ranked by the program, you can't match there.
 
On the other hand, if you interview well, if you show them that you sincerely want it and your records show that (you gave 110% on an ENT position and got good letter, etc.) you may be ranked highly or high enough to have a shot. Like you say, sometimes applicants who are of high caliber get picky (understandibly) and forget about the little programs and this may be your shot at getting in. Having just gone through the match and talking with classmates, I've come to realize that if you really, really want something and go after it with the same zeal, and your open minded as to where you'll end up for residency, you'll be surprised at what specialty you might land. I think it's worth a shot!

The two ENT spots that were open this year during the scramble were research positions...so essentially, not really "open" ENT spots.
