matching forms

Hi everyone, I need some advice. I have used the search feature already but came up with conflicting results.

anyway, I visited my organic chemistry teacher on the last day of her summer class today to get my last letter of recommendation. I watched her seal the letter but it did not occur to me at that point to remember to include the letter of evaluation matching form inside the envelope. I just realized this right now. I plan to send this letter with other documents in one big packet very soon to the processing center. With my transcripts and other LORs, i have made sure that the matching forms were sealed inside with their respective documents in individual envelopes.

What do you think I should do at this point. Ask my professor to send in the LOR herself or just send in what I have? Is it absolutely required for the LOR matching form to be included with the LOR because the instructions make it seem like it is only necessary if the school or professor sends them independently.

Since you will be sending all the LORs and transcript in one big package and that other documents have matching forms, then you don't have to get a matching form for the LOR in question. However, if you want to be safe, you can just write a LOR matching form yourself and put it inside the big package.
 
Although i'll be applying next summer, i recieved a LOR from one of my professors. I'm not sure how the matching form from AADSAS would work. Would i wait until the applications for the next cycle come out next summer through AADSAS and then fill out a matching form myself?
