I never had the chance as an undergrad to study abroad, is it possible that I could have that chance in medical/graduate school? (would you be considered in medical or graduate school if you are studying for an MD/PhD?). Or should I take some time before I start my further education to spend some time abroad, without the study part?

I'm not sure if I understand the first part of your question. I would not advise you get your MD/PhD abroad if you are able to get into a US program.
Some Options:
1. Do some research abroad or something else you like before you start the program
2. Most medical schools allow you to do away rotations at international sites in your fourth year

Forgot to mention the NIH OxCam program which is an MD/PhD program that would allow you to do part of your PhD work abroad in the UK.

Also many labs that have collaborators in various parts of the world (and are well funded) will be open to sending their students to learn and work with their collaborators on a project for months at a time. I have multiple PhD friends who did this.

Possible, sure. I spent 6 weeks in Guatemala during some vacation/elective time. You can probably find some similar small blocks of time, though it's taking out of your research time or other clinical elective time to do it. The Oxcam or having a lab to send you over for "collaboration" is uncommon and I wouldn't expect it, though there's nothing wrong with seeking it out if that's what you really want.

My advice is to just do study abroad how and when it makes sense for you. If you can do some before starting MD/PhD, why not? I spent a summer in Spain as an undergrad. It wasn't "research time" but you gotta live at some point.
