I have been looking into MD/PhD programs. Many offer free tuition and a stipend. Does this free tuition mean only for graduate school or for both Medical School and graduate school?

It's the whole thing. You are postponing a physician's salary for quite a lot of years (for some folks longer than others), so to compensate for the huge loss, they cover your expenses. You don't go this route for financial reasons -- it's a suckers play. You do it because you have a particular career path in mind.
 
My ultimate career intent is medical research, so I am guessing this would be a wise choice. The free tuition is just a bonus.

This is something you need to be very careful about when looking into MD/PhD programs. While most programs will waive your tuition and offer you a stipend for the entire length of the program, there are schools out there that will cover you only for the graduate years (and the last 2 medical years if you have secured grant money for yourself). Additionally, some schools will offer you free tuition and a stipend but not cover other valuable things like insurance costs.

In general, the MSTP programs will fully fund their students, meaning that they offer free tuition, a stipend, and covered health insurance. As far as non-MSTP programs are concerned, many will offer full funding but some will offer it only to a few of their students (i.e. the most competitive students) each year, and some will not offer funding for the medical years at all. There is a thread on this somewhere in the MD/PhD forum that may be helpful to you, and this information is usually available on program websites. Good luck!
