I have a meeting with a professor and I plan to ask him for a LOR. How should I go about asking? Do I ask him in the beginning of the meeting or talk with him a bit and then pop the question? What should I make sure I tell him so that he can formulate a strong letter? I don't know him extremely well, but it's all I have at this point, so I just have to go with it.

Just ask him. Ask if he's able to write you a strong letter of recommendation. Be sure to say strong. Offer to send him your personal statement. He may have additional questions for you.

You should ask "Prof. X, do you think you can write me a strong LOR for my med school application?"
