Heard (and seen when I was a med student) lots of things. All of them bad.
when i went in for an interview for IM during the scramble, the resident and faculty told me that they give the in house residents preference for their fellowships. and since you will already have contact with each of the fellowship's director, you will already know what they expect and want in the application for that fellowship. and as long as you meet all those requirements , shouldnt be a problem getting those in house fellowships.
