All my diplomas have my middle name. Never even thought about it.
FWIW, most other physician diplomas I've seen on office walls also seem to have their full names.

Definitely. Middle initials convey more professionalism, and avoid possible name clashes.

The name on your diploma isn't that big a deal. Do it how you prefer.

My undergraduate degree has my middle initial on it and my master's degree has my middle name on it. I don't think it matters-- it's personal preference.

make it say whatever your undergrad one said. Reflecting back on it I don't think I really had a choice.
