Min/max fellowship call question

Are there lower and upper limits to the amount of call that one takes in a pediatric or cardiac fellowship?

I assume ACGME-accredited programs are bound to rules similar to those seen with residency programs, but if anyone knows specifics I'd appreciate hearing them.

ACGME fellowships are bound to all criteria of the 80-hour work week, just like residency programs. 10 hours between shifts, 24 + 6 hours on call etc. We cannot negotiate for additional call.

Non-ACGME fellows take whatever call they can negotiate when they sign their contract. There are no restrictions on their hours except for whatever they negotiated for.

I am an ACGME fellow and all of my call is home call. I do 7 days on, 7 days off. On my call days I stay until the last cardiac case is done and come back in for any overnight emergencies or transplants. On my non-call weeks I usually leave after the last case or around 2000 whichever comes first. If I stay past 2300 on a call or non-call week I get the next day off... I don't stay past 2300 nearly enough.

So if you stay until 2300 do you not come back the next day until 0900?

No. If I stay until 2300 I have the next day completely off. If I stay until 2230 then I come in the next day at 0830. I don't know why it always takes so long to extubate people when the case finishes at 2230ish.

This is the same arrangement for our cardiac attendings. We need a definitive and fair way to determine whether or not to work the next day since it is unpredictable whether we will work overnight when on call.
