Minnesota Supplemental Application...where is it?

They post the link to the site where the application should be available, but I don't see it anywhere. Anyone else having this problem?

Programs and Student Admissions > D.D.S. Program > Admissions Information (right hand side) > Supplemental Application 2010-2011 (under related links on the right hand side again)
