I read an article, perhaps over the summer - maybe earlier, that stated what I wrote in the title: A study showed that mistakes made by Residents may be underreported, and a new system was being implemented somewhere (maybe cleveland clinic? maybe not) that fix that problem (perhaps anonymous reporting, perhaps some other method to encourage reporting).

I don't remember where I found it and I'm having difficulty googling it - anyone know what I'm talking about and have the source?

This isn't specific to residents, but there was an article in the NY Times about doctors becoming more open with their mistakes as a way to mitigate lawsuits for the hospital
