Do you really have to ask Uro/GI/sleep questions for everyone in order to get all the points? It seems like those ROS systems would only need to be asked if it is relevant to the CC; otherwise it seems like a waste of time, I mean, if you ask those, why not ask Resp, cardio, and other ROS questions?

For describing pain, I like the following "LIQUOR" mnemonic better - if only for the fact that it preserves the spelling of "LIQUOR"

L = location
I = Intensity
Q = Quality
U = Upsetting (aggrevating) factors
O = Onset
R = Relieving factors and radiation.

Excessive Guilt
Suicidal Ideation

And my bf and I made up - ID PSYMISTS (a bit silly - but it does the job in helping me remember)
