A patient comes into your pharmacy for a refill, he is around 50, and has diabetes and hypertension. You notice the last time he refilled his prescription was 3months ago, what do you do?

I am assuming that this is a late refill.

If the original date of the prescription is a few months old, (which it most likely would be, since his last refill was 3 months ago) I would call his doctor and confirm whether or not we can just refill his med, or if he needs to go in to have a check-up with his doctor.

Well, if you gave him a 90 day supply 3 months ago, there is no problem.

There are lots of explanations for missed refills besides noncompliance. He could have gotten samples, filled elsewhere, or have been instructed by his physician to take a lower dose. You need to talk to the patient for more information before you decide upon a course of action.
