Monoclonal spike on protein electrophoresis (urine analysis) is indicative of multiple myeloma b/c of excess light chains.

Is it indicative of any other cancers or diseases?
Would minimal change disease would also produce monoclonal spikes since albumin is the only protein lost??

thank you in advance.

MGUS also has monoclonal spikes often of IgA or IgG (with a much lower proportion of IgM) on urine analysis. MGUS can be a precursor to multiple myeloma (I think the stats are around 20% will get multiple myeloma).

Waldenström macroglobulinemia and amyloidosis (lambda or kappa chains) may also show up with monoclonal spikes.

http://www.aafp.org/afp/20000915/1333.html is a good article that reviews proteinuria.

I think it depends on what spikes the electrophoresis is looking for on a urine sample (i.e., light chains, which is more typical). Albumin is usually picked up on a plain UA strips, whereas the amount of protein lost with other proteins like the light chains is not as accurately determined.

Yea dude, MCD is a monoclonal spike disease. Cuz, y'know, albumin is AN IMMUNOGLOBULIN made by a SINGLE CLONE of PLASMA CELLS.

You should probably read your textbook a bit, or contribute to the forums, before you ask people to do your homework. I'm not gonna count but I bet 90% of your total posts are this spate of laziness or you defending it.

I think the bigger question here is... why the hell would you do a urine electrophoresis in a post-viral illness kid with nephrotic syndrome (the typical MCD patient)!??! 😕
