I very recently decided I'd like to do an MSK fellowship. I just started my R3 year. Is it too late to realistically get a spot at a decent program?

No, it's not too late. If you are quick you could get applications sent out to several programs soon. If you miss this interview season, there are frequently spots opening up. Keep an eye on auntminnie. Programs have unexpected openings quite often because people who verbally commit 2 years in advance later change their mind and back out.
