OK, so how I see this is that you forged a faculty member's signature to expedite some paperwork after being unsuccessful in reaching the faculty member over several weeks time. I think that you just have to explain it that way and that you realize now that the need to get the paperwork approved by the registrar did not justify forgery.

No one doubts that you did the work and there was no accusation that you were falsifying your hours, only that you were unable to get a second signature and took matters into your own hands (quite literally) to get the thing put to bed.

I think that at this point you have to apply to the schools that you are a fit for based on your application (see WedgeDawg's assessment tool) and hope that at least some adcoms are willing to be lenient with someone who had some youthful impatience. I'd suggest applying to 20% more programs than you otherwise would. So if you would have applied to 15 (relatively low number) make it 18. Plan to apply to 20 schools? Make it 24.
