Say someone has onychomycosis and they cut their infected nail down to the cuticle and file away all visable nail fungus left under the nail.... how is it that the nail grows back being infected again?

Even then, there is no guarantee of getting rid of the fungus since the efficacy rate of the oral antifungal medications is about 70%. In addition, you should be aware of the potential side effects and drug drug interactions.
