NAPLEX Texas board application procedure

I am a foreign pharmacy graduate. I passed FPGEE and TOEFL ibt. I want to apply for the TEXAS board NAPLEX exam. After reading the TEXAS board website, I found that the board will go through my education credentials and determine if I am eligible to sit for NAPLEX.Only after getting an ATT number, I can apply for NAPLEX through NABP. I want to send the necessary documents but unfortunately I can't find the necessary applications or forms on the website.I'd really appreciate if anyone could direct me to the url of the TEXAS board website where I can find the appropriate forms or mention which forms need to be filled out or sent for determining my eligibility.
