I actually just recently finished my senior honors thesis on this very topic! Although it's a few years old, there is a 2000 report of the U.S. surgeon general regarding oral health in America (available as a pdf file online). It's 300+ pages of all sorts of information relating to this topic. I found the majority of links to be dealing with children, as the worst disparity (and darkest future) regarding poor oral health is seen in terms of this population group. Basically, a good old internet search yielded the best results for me (with a few hits from PubMed as well). The only major problem I ran into in collecting sources was gathering "up-to-date" information, as many of these oral health status reports (like that of the surgeon general) are published once every 10 years.
