NBDE Part 2 Score Report- Entity Name/Contact Name

Can any one shed some light on the score reporting for part2 NBDE to Universities.

I am not sure what to mention for
1. Entity Name
2. Contact Name

Should these both be name of the University & Department ? 😕
for eg. UCLA School of Dentistry!
