Q: 45-year-old woman with 4-month joint pain, muscles aches and fatigue. Had a renal calculus 6 months ago and had cholecystectomy 1 year ago. Appears fatigue, depressed, there is mucosal pallor. No masses palpated in neck. Labs: erythrocyte count 3 million, K+ 4, Cl- 106, Ca2+ 13, phosphorous 2.7, alk phos 125. Technetium-99m scan shows 1.2 cm nodule in neck. Mutation?
A: MEN 1

Need help understanding how this is MEN1. Thyroid nodule- yes. Renal calculus- due to parathyroid dysregulation of calcium. But I do not understand the role anemia and cholecystectomy play. Please explain, thank you in advance!
 
Acute cholecystitis is common in females around this age and so is anemia, I don't know if it's related but you should get used to this collaterals wether in exams or in real life.

What else is it going to be? Get used to these kinds of distractors. Patients with MEN1 get heart attacks.

Don't let them fool you. You know it, so pick it, and move on.
 
Gahhh, overanalyzing yet again! Thank you all, my first thought was MEN1 but the fluff threw me off. I am trying really hard to not get distracted by the extra info, but I can't seem to move passed it.
