I took NBME 4 and i answered 78% of question correctly (39 wrong total) and my 3 digit score was 212.....is this comparable to the real exam, i mean would i have to answer 78% of the questions correctly just to bearly pass....this doesn't make much sense since the avg step 2 is 237 and that would mean answering like 90% of the questions correctly

Good news is i have about another 23 days left to study and my practice exam scores are trending up each week but im confuse as to how the NBME scale/scoring correlate with the real thing.

The NBME prediction tells you what a similar performance would be scored as on the real exam. Obviously your score will depend both on how many questions you get right, but you cannot predict that you will score 78% on an easier or a tougher test. At your current performance it is likely that a test similar to the NBME4 will see you score in the ballpark of your prediction (if you take it now).
