I just took the NBME exam 1 and had some questions about their questions:

In the tutorial, the guy with orchitis how do you know if that is chlamydia or gonorrhea?

For the infant picture with that big thing on his head-- WTF is that?? A myelomeningocele??

What was going on with that teenager with the crusty stuff all over his nose? Was that eczema? What was the diagnosis?

For the old lady with postural hypotension that was caused by amitriptyline, right?

How is cleft lip inherited??

How do you approach the vertebral angiogram question?

Is there any resource where you can review the NBME shelf exam questions? I can't seem to be able to review my answers. Has someone posted a thorough Kaplan-like discussion here or somewhere else?

I can answer a couple... the orchitis is gonorrhea, as it is much more likely to cause any type of "penetrating" infection like orchitis, prostatitis or epidydimitis. amitryptiline causes orthostatic hypotension; the tca's all have untoward anti-cholinergic effects. cleft lip is multifactorial, i believe. no clue about the crusty kid, that thing baffled me. in retrospect, maybe it was the atopic triad? not sure... lastly, i don't remember the vertebral angiogram.

hope this helps.

What about the one with the doctor in the elevator? I thought the right answer was to remind the residents that patient matters are confidential. Also, what about the mad patient? I put "You seem to be upset, is there something bothering you" as the answer but my other choice was "What brings you here today?"
