Is your observership here in the US?

Do you have any US clinical experience?

Any US letters of recommendation?

Do you have to limit yourself to only south NJ and PA?

hmmm. without knowing anything about you except what's in your post, it would be impossible for anyone to predict your chances. however, you asked for advice on how to improve your scope...

the major thing would be to apply to programs in more than just that small area. if there is some reason that you cannot possibly leave that area, understand that this could mean you don't get a residency placement. You might, again I don't know your other credentials... but a big thing that you can control is how many programs you apply to. applying in less desirable areas gives you a better chance.

people will want to know why you have waited this long since graduating - 2001 means 7 years out... were you a practicing physician somewhere else?

and, as blade already said, US letters and US clinical experience will be important.

Please accept my apologies as I do not mean to offend. If your post is indicative of either your typing, spelling or grammatical skills, please have a native English speaker who is well-written review your application and PS before submitting it.

As others have noted, letters from US faculty are key as is a reasonable explanation of what you have been doing since graduation.

I recommend getting your ECFMG certificate and passing Step 3 prior to application. At the very least please do the former.
