Need help on pharm. error essay

I'm doing an informative essay on pharmacy errors and I'm having difficulty tracking down certain info. Wondered if anyone had any good links to statistics or information. Not news stories specifically but more information geared. I just spent about 30 minutes trying to track down your odds of a fatal error, finally worked it out but my goodness I don't have this kind of time to do this.

Thanks for any help or info anyone can pass along to me! Also, if anyone knows where I can find any published reports done I'd really appreciate that as well.

A really good source for information about pharmacy-related errors is the Institute of Medicine, a branch of the National Academy of Sciences. Here is a link to one of their reports on medical errors released in 2006, it's got some good numbers and facts to cite:
