Need opinion-trying to prep PBL exam

I have my first PBL case-study type exam tomorow... we basically go step by step through a hypothetical patient.

Here's my question: I know the "patient" will be pediatriac... what types of diseases do you think might come up?

We have already had (in practice tests): Myasthenia Gravis, Cystic Fibrosis, MI, familial hypercholesterolemia, and chronic mylogenous leukemia...

i was thinking it could be endocrine.. since thats a major system we haven't touched on yet...

what do you guys think?

it was glycogen storage disease Ib

if u want i can summarize the step-by-step symptoms/labs we got, it was pretty tricky crap esp. if you suck at biochem like me...

I dont have it with me right now but here is basically the first page of our exam.....


4 mo. white female present with fever (100-101) lasting two days. Has been listless, restless and irritable. SHe was a healthy, full term, normal weight baby, but had severe hypoglycemia a few days after birth, at this time she also had sepsis, was treated and released. Since her fever has started she has had a reduced interest in eating, loose, watery stools and vomiting.

The mother is healthy, although aunt and grandmother both have Type II diabetes. the mother reports being occasionally tired, but has recovered from the delivery fine.


Physical: i dont remember the whole thing but here is the gist of it:

red inflammed left tympanic membrane, bitemporal narrowing, fine hair, prominent cheeks, increased subcutaneous fat, thin extremities, below normal weight, chubby face

normal lungs/sounds, heart normal

respirations 35, palpable firm liver with no nodules, no palpable spleen, normal heart rate, temp elevated (38.5);
she was alert
that was the end of our first bit of info, i dont think im forgetting anything; from this we were to form 3 reasonable hypotheses and support each one with 3 findings and explain why our support could be supported by our hypotheses (kind of circular, i know, whatever)

tomorow i'll add our second page of info, or later tonight if i'm bored...

and ps im a lowly ms1 so if i sound like an ass talking about this stuff its probably because i kind of still am...
