Need Special Advice Regarding School List

Basically, I'm applying this cycle and am going to have to report an IA from 4 years ago (during first quarter). Posted my stats on the weekly thread but was wondering if anyone who was successful/has been in a similar situation could shed some light on wtf I should do.
Note: I have served as a volunteer in the office (~70 hours) who gave me the sanctions (academic integrity office) and have a letter of rec from the academic integrity supervisor to show growth from the experience
