How many of you are planning to stay at the institute that trained you, and how much of that is motivated by personal versus academic ties? Considering that 30-50% of MD-PhDs do stay at the same place, is it really career suicide or a recipe for success?
 
While I can't speak for graduating MD/PhDs, I can say that I've always been a big fan of being academically promiscuous, so to speak 😉 . I think you challenge yourself and learn more that way. You might have heard the phrase "intellectual inbreeding" in reference to staying at the same place for various phases of academic study and training. A pro of staying at same place is that you will have already established connections and built support networks that might be non-existent elsewhere. As MD/PhD's, we have many opportunities to gain new perspectives during the transition phases from undergrad -> md/phd program -> residency -> fellowship/post-doc -> job(s). I have heard from various different insitutions, however, that often in the later phases people stick to the same place due to personal ties (e.g., spouse and/or children). Sticking around might not be total career suicide if you are planning to stay there permanently anyway. If things are good, then why leave? In addition, sometimes sacrifices have to be made for family and other important things in life.
 
According to my PI (a PhD), staying on as faculty at the same institution that one trained at is viewed negatively when evaluating grant applications, as it is considered to show a lack of independence. Also, I have heard from many people that big changes in salary/position often occur only when one changes institutions/jobs (true of many fields, not just academic medicine). Having said that, if I find a place that I and my wife love, I would have no problem staying there (in fact, this is what we are presently planning to do). There is something nice about knowing that one will be in a given place for a decent amount of time...
