Having problems with our NT2000 when doing 4 simultaneous lesions with 16 ga cannulae. Only one cannula reaches desired temp while the remainder stay at 45 degrees. Is my recollection correct that others have experienced this??
 
I use that with 18g and don't have that problem.
 
If I place 3 Nimbus needles I usually notice one won't reach desired temp til others are finished. I don't typically place more than 3 18 gauge cannulas for traditional RFA so haven't had this issue.

I use the same machine. I also use 16 ga cannulae.

Here is the likely situation; you used to have no problem running 4 simultaneous lesions. Then one day your machine cannot get all 4 up to temp. You are pissed about all the time you are wasting. You send it in for testing and repairs.

This was the issue for me:

At some point my NT2000 had a firmware update. Stopped doing 4 simultaneous lesions after that. Would have same problem as you.

Sent machine in; checked out ok.

Complained a few times. Finally got an answer.

The stupid government got involved and messed things up. The machine is supposed to be limited to 55W max output. In order to bring 4 cannulae up to temp simultaenously, the machine often (usually?) needs to exceed 55W.

Neurotherm allowed that to happen for a long time. Then governement got involved or Neurotherm's lawyers decided to ruin things or a combination thereof. Neurotherm issued a firmware update preventing the machine from ever exceeding 55W, even for a short time. This essentially killed the ability of the machine to bring 4 cannulae up to temp simultaneously. Neurotherm did not bother to tell physicians this important fact, most likely because at least some competitors did NOT hamstring their machines in this fashion and they did not want to loose market share.

I also have a Baylis machine, no problems doing 4 simultaneous lesions....

Anyway, finally was told this story by Neurotherm technician.

The solution is to enable the "staggered" lesion mode, which will bring cannulae up to temp one by one, spaced 30s to 1 min apart, or something like that.

It is stupid.

Most likely this is your problem, see if you had a firmware update or they swapped your machine.

True. Moving the pad just a little bit closer makes a difference bigger than you'd think.
