New ACC/AHA Guidelines! Treatment of Cholesterol to Reduce CV Events

Just FYI, the newest cholesterol guidelines to reduce cardiovascular risk were just published today: http://my.americanheart.org/profess...CCAHA-Joint-Guidelines_UCM_321694_Article.jsp (among three other guidelines)

There are some sweeping changes. Fortunately, they've decided that LDL goals were not evidence based. Instead, they provide strong recommendations that the four following groups should be evaluated for statin therapy:
  • Individuals with clinical ASCVD (atherosclerotic cardiovascular disease)
  • Individuals with primary elevations of LDL ≥ 190 mg/dL
  • Individuals 40 to 75 years of age with diabetes and an LDL 70 to 189 mg/dL without clinical ASCVD
  • Individuals without clinical ASCVD or diabetes who are 40 to 75 years of age with LDL 70 to 189 mg/dL and a 10-year ASCVD risk of 7.5% or higher
The guidelines also no longer recommend Framingham for 10-year CV risk assessment. Instead, they developed their own risk assessment tool (called the Pooled Cohort Equations). They do provide an Excel document for calculation, but I've shamelessly created by own online-based one, too, for convenience: http://clincalc.com/Cardiology/ASCVD/PooledCohort.aspx

Very nice post. There was a ACC article years ago that spoke about how the plaque stabilizing effects of statin was probably more important to clinical outcomes than say the actual LDL lowering effect (surrogate). Unfortunately EBM practices take a while to implement.

I jest. I was a little disappointed to see the Framingham go, but hey, EBM...

Certainly a huge difference from the old treatment guideline. Now all the sudden my cholesterol is no longer an risk factor according to this and I don't need to be taking lipitor. Cool! Let's eat!
 
So we've been wasting money for statins to treat LDL these past year? It is clear to me now a a whole business of guidelines is to get money from people.
 
The new parameters are represented in a looooong equation that involves natural logs, coefficients, etc.. The fastest way is to download their excel chart and plug in the pt's profile. LDL value is no longer considered in calculating the 10-year risk.
