According to the ACGME RRC, first year derm residents can't take call or see consults starting 7/1/11.

Does anyone else think this is absurd?

Yikes... thats some crazy stuff.. and they can only see 1/2 as many patients as 3rd years?

I love how the ACGME can just make rules with no consideration as to how they will impact work-flow. They justified changing all of the rules for interns with citing some data backing up medical errors with long hours, etc. I would like to see how they are backing anything up with the changes in Derm.

Maybe I'm wrong but I just dont see how those guidelines can be met in most instances (i.e. where residents see the vast majority of the patient's and write all of the notes/scripts/etc), Would love to hear what other people think about it....

I wonder how our program will accommodate those changes?

Traditionally, our 1st years are spared from consults but I have no idea how they can avoid taking call.

And seeing half as many patients as the 3rd years? The 3rd years would likely revolt!

I will say that there are programs out there where the residents do serve as the majority of the workforce, seeing the vast majority of patients (obviously with attending supervision) but also completing the note (where the attending just has to sign below) and filling out all the prescriptions.

Good news...actually I'm reading the new rules as stating that the PGY-2 must see AT LEAST (not only up to) 1/2 as many patients of the PGY-4. So it doesn't seem that they can only see 1/2...rather it looks like they'll see at least 1/2 the "clinical load" of the seniors as a minimum.

Hey skin tag, I couldn't find the call/consult requirements for first years. Could you point me to the site where you saw that? We don't let the PGY-2 take call or consult at the beginning of the year but they do start later in the year.

Not to mention these new rules come less than 6 weeks prior to the beginning of the 2011-2012 academic year. I imagine most programs will need to significantly revise their schedules to accomodate this nonsense. This is obnoxious and extremely inconsiderate on the part of the ACGME. The arrogance is stunning. Needless to say, the burden will mostly fall on the 2nd years to see consults and take call. I attached the PDF which states the new rules.

Finishing out my first year, I have already done 2 months of consults and 10 weeks of call (have 2 more of that)...

With that said, what is the reasoning? Sorry, but I am too preoccupied with studying and moving right now to look it up myself.

Just finished up a malignant prelim year where consults (aside from ID and of course Derm) were some of the most downright nasty people I have dealt with on the planet, worse then telemarketers with learning disabilities, I am sooooooo looking forward to consults...I think it will be one of the things I like most about derm!

HAHA....Sorry to hear that Izzy. It's too bad because out in the community that kind of nastiness will keep food off the table....but in the academic centers sometimes the consultants are pretty darn rude (primarily cuz they are fellows and residents). Well remember that and make sure you don't treat the poor interns that call the same way...then again, like you said, we dermies are pretty nice anyway!!

And yes, it's awesome to not have to call all the consults anymore...
