New york state how to request another copy of pharmacist license

I'm having trouble finding the forms to fill out to get another copy of my pharmacist license for new york state. If someone can please post the link to request another copy of the license I would greatly appreciate it.

License or registration certificate? My reg cert lists "
If any of the information is not correct, please contact us at [email protected] or
(518) 474-3817, Ext. 410" so I'd start there

Go on the education department's website. They provide instructions on how to get a new paper copy. Whenever I moved, they sent me a new copy of my registration but not my license (makes sense since license doesn't have address info). I believe you have to pay ten bucks for a new paper license.
