are there any calculators online for NICU?
I looked at some calculators for Vancomycin and Gent but they are all for adults, not for NICU

Harriet Lane and Pediatric Dosage Handbook were sources I used during my rotation there. PDH is through Lexicomp and generally if you have a sub for it, you can find the information online, although I've not been in a peds or nicu ward that didn't have a couple of copies around.

One that I didn't use was the Teddy Bear Book (ASHP), mostly because it was only IV. Nelson and Lange have books that are more medically orieneted but were interesting reads.

I am actually looking for gent/vanco dosing adjustment calculators.
Let's say I have this peak and trough and I need to figure out how to adjust the dose to obtain this or that trough...
Does PDH help with dosing adjustment?

No. You do it like you were taught in school. I wouldn't trust a calculator with a baby's kidneys.

you mean to go through all the Cpmax calculations? I worked in 2 hospitals, both hospitals use calculators, no one goes through the whole equations by hands....
But they have PK programs and I don't...

I work in a major academic medical center and we do them by hand.

Maybe if you have a legit PK program, but those aren't readily available as resources.

I calculate it by hand as well.
