NMS Casebook Dated?

For those who've written the surgery shelf: I'm using the NMS Casebook, and I'm a little taken aback by what I would consider as being dated information.

For instance, in the management of DVT/PE thrombolytics (tPA et al) are not mentioned (only heparin is) and, as another example, in the management of medically refractive PUD highly selective vagotomy is the preferred option (whereas Uworld disagrees).

Anyways, just wanted to hear what some other people thought.

Thrombolytics are rarely used for DVT/PE though, and I've never seen that be the right answer on an exam. The reason being that most major PE's kill the patient before they get to the hospital....And those patients who develop PE while in the hospital, often do so after major surgery or major debilitation and thus are rarely candidates for thrombolytics. The information not in NMS casebook that you should probably no are the expanded indications for carotid endarterectomy because that has been in the literature a lot over the last 5 years.

As far as treatment of refractory ulcers, the only thing you need to know for the shelf is zollinger-ellison, the surgery shelf has very little as far as detailed questions on preferred surgical procedures....You just need to know the indications for surgery.
 
Thanks for the advice; there are a few nit-picky things that make me wonder still, however, I agree with you guys in regards to it being a pretty solid book. I'm reading it now as I'm typing (i.e. procrastinating). Hopefully it'll serve me well.
