are you guys seeing this?

I'm tired of commuting over an hour a day to work to scrape together enough $ to pay my child support to avoid getting thrown in jail, so I approach a hospital 25 minutes or so away. I inquire about the possibility of getting in the rotation to see patients there or find out what's going on with their current contract.

They tell me that so and so has the contract. Who I say?

it's not even a psychiatrist. So I contact the group(not a psychiatric group btw) that does have the contract to see if I could see some of the patients(for a fraction of the pay of course since I dont have the contract)and they've repackaged it and are letting a couple psych nps do it through telepsych(apparently 25 minutes is too far to drive for them). wtf? The damn hospital just accepts whatever the most attractive bid is....who cares who is doing the bidding...just so the work eventually gets done by someone.

what's to stop jose at Home depot from bidding the next contract? Then he can just hire some people to swing by and cover it(or telepsych it) and lop off some of the top.

frustrating.

The hospital is probably too busy focusing on ortho and cards. From a financial standpoint, psych never saves the day.

jose at home depot is an undocumented immigrant who has lived here since the age of 2. an ICE agent will stop him and deport his as$ back to mexico, that's what.

but no im not seeing this. if you cant move there is plenty of work you can do right where you are. I know of a telepsychiatry gig that pays 250k + benefits, I saw another telepsych "hospitalist' 7on/7off that pays 270. you can do utilization review. you can be flown in from out of state to work a weekend at a hospital and make 7k a weekend. a slightly above average psychiatrist can find a way to be well compensated even in areas with a glut of NPs. a slightly below average psychiatrist can find a way to be even better compensated by dishing out delicious treats like Adderall and Suboxone

I'm wondering if the reality is ever going to set in for Dr. V that maybe, just maybe, the problem isn't with every person he comes across and the state of medicine and the field of psychiatry, that maybe there is another common thread in all his problems.
 
This reminds me of the guys in the the Anesthesia forum talking about CRNAs. Once hospitals figure out that there's a nurse out there who will do 'zactly the same thing we do, for less money and with less attitude, we're in deep trouble.

Nope. This job market is still desperate to hire psychiatrists. I would think you could easily get some tele psych gigs that are better than what you seem to be able to find in the local hospitals. Any reason you're not doing tele psych?

At least psychiatrists have the option of working outside the hospital for cash. I would expect that most people who are willing to pay cash to see a psychiatrist would not be satisfied with seeing a NP.
 
Speaking as a cash-pay patient who sees a psychiatrist for therapy only in a large East Coast city, you could not be more right -- no way I would go to an NP. I would never even consider it. My worst therapy experience was with a non-psychiatrist, never again.

Not that I would consider leaving my doctor -- I am only talking about a situation where he retired or moved!

Some are loyal like you are, some don't care and some don't want treatment. Some just want whatever meds they can get.

Have you considered opening a private practice so that you're no longer someone's lapdog, vist? Or do patients not take kindly enough to you that you believe you could be successful in private practice (and I say that not as an insult, but as an honest self-evaluation thing, some a miserable psychiatrist is unlikely to have satisfied patients)?
