So I realize the DO cycle runs longer and October is getting slightly late to be applying, but I applied mostly MD (17 schools) and received 4 II so far. I was thinking about applying to PCOM, LECOM, and NYITCOM, but I was reading these schools fill their classes much earlier. Is it worth applying to them at this point?

I wouldn't bother applying to DO with 4 MD II...but if you want peace of mind I guess. And with your stats, no it's not late AT ALL.
 
The no DO letter will hurt you. I would not apply. Your stats are sufficient for MD schools, I'm surprised you don't have more II. Did you apply appropriately?

I only applied to one reach school. All others my stats were above the median of the matriculants. No rejections yet.

That's good. If you're really worried about not getting accepted, you'd still be okay to apply now. I had my initial application done, but didn't add schools until October for DO and ended up with >10 IIs with mediocre stats. Just make sure you find the schools that do not require a DO letter.

You have an excellent application for MD, and even have 4 II's. Isn't a 512 like a 32 on the old scale? Save your money. Heck you could get into lecom if you applied in January.

As far as the DO letter thing, if you want to make use of your time you could shadow a DO for the experience. If you end up needing a DO letter, you got one. If not, no big.

non-trad
MCAT = 512
GPA = 3.8+
multiple first author publications
generic ECs
excellent LORs
only 6 hours shadowing a DO (no DO letter)
30 hours shadowing MDs

The average MD matriculant, according to aamc, receives 3 II. You have 4 and it's still October. Idk why you're so worried right now. And did you interview at any of those schools yet? If so, you'll probably hear a decision within the coming weeks since we're approaching Oct. 15. You're in a great position. Save your money and don't apply.

What would you consider "mediocre" stats? And will no DO significantly hurt chances? As long as you can explain why DO and maybe some shadow experience, you dont really have to have a DO LOR. Obvi it's better but not required

Are you asking or telling?
From what I know, it sounds like it is based on the institution but I have not heard much talk here on SDN on this.

When I was applying, the LOR requirements were very strict. If a school says you need "X" letter, I was not given an interview until I sent it to them.

Not so much worried as hedging my bets. I am going to lose my job next summer and it will be incredibly unlikely I will have the flexibility to go through this process again....not to mention the finances. I don't really want to start any later as well as I'm not getting any younger.

And yes, I interviewed at one. I was planning on submitting my DO app if it was a reject.
