I finally got professors willing to write my LORs and all of the schools I am applying to have pre-printed LOR forms on their websites. However, the Nova Southeastern University website has no such form. Do any of you have any experience with Nova? I am just wondering if they send you the form when they send you the supplemental application or if the recommender just has to write the letter from scratch.

Most schools such as Mercer & Samford strictly require to fill out their evaluation forms. Some schools such as Nova and Campbell accept either the writing or the evaluation (form). You should ask your professors to fill out the forms, but with your co-worker (pharmacist), ask for the writing. I experienced that most professors just write 1/3 of a page with general comments! Nova will send you a package, which consist of a supplemental application and 3 evaluation forms, after they receive your application from PharmCAS. At meantime, you should ask them to fill out other schools' forms and tell them to make a copy for later use.
