Just wondering - I know that not following through with your matched program is a match violation, but suppose one were to match in a specialty requiring a preliminary year and decided to continue on in the preliminary year program as a categorical without going to the matched program which was to follow. (This is assuming, of course, the IM or Surgery program made a space for you.) Anyone know what would happen? Anyone know someone who has done this?
 
Doubt it would happen. From what I hear usually not abiding with the match outcome is career suicide and you will get blackballed unless the other program oks you.
 
Not career suicide.

I have a friend who matched into rads, started a medicine prelim year and realized that he liked it too much to leave. Gave the rads program plenty of notice, asked for a spot at the IM program (and got it) and everything worked at great. It's totally a do-able situation, if you're honest and communicate well with everyone. I have to confess I don't know what the "rules" are about this.

I think this is the key.. As long as your initially matched program ok's it then thats cool. Maybe it is cause I am tired I mistook the meaning of the OP. If you get the OK then there is nothing to worry about. But you def have to give them some warning. Something like Rads surely has a ton of people willing to take that spot..
 
worked with an intern last year who was doing IM prelim and had matched into anesthesia. He found a rads spot in February of his intern year and called the anesthesiology program to see if he could get out of his contract. They said sure no problem and he signed a new contract with the rads program. In the beginning of april the rads program sent some kind of letter to NRMP to let them know what had transpired and NRMP freaked out. The intern was still in the process of faxing a million things to a million different people at the end of April when I left the service. I am guessing it worked out for him because i bought a bunch of anesthesiology books from him and he said he was going to call me if he needed to buy them back and I never heard from him.

Just one experience and its not even my experience, so take it with a grain of salt. The moral of the story is that most programs don't want a miserable resident so they will mostly let people out of their contracts....the NRMP is a grumpy dictator.
 
The program you matched into has to release you. I believe you have to apply for a waiver from your match committment with the NRMP which is usually granted if the releasing program oks it (check on their website, they have a short synopsis on all the waivers they granted for the last match).

Timing is key, don't call them a month before you start. If it is a competitive specialty and you give them 6 months to take one of the people off their waitlist you shouldn't run into problems.
