Hi anyone have a copy of NYCOM secondary that they can scan in for me. I have an interview in a couple of days and just realized that I did not receive the secondary application in my packet. The admissions office is sending me one, but I am afraid it may get here too late. Can I just get the questions if you can't create a file. Thanks! I think I'll owe one of you my fingers or toes for this

I recently completed the NYCOM secondary. Unfortunately, I do not have a scanner. The form will not take you more than 30 minutes to complete. The longest section -- about 5 lines -- asks "Why do you want to go to NYCOM?" Other questions ask about your research experience, work experience, extracurriculars/hobbies, and clinical experience. Judging by the amount of space they provide, they're looking for concise lists of things, not paragraphs and essays.

I wish I could help you with the scanning but here are the list of questions...so at the very least you can just have those answered on a seperate sheet to be attached to this when you get there for the interview.

1. List all paid work experiences from highschool graduation to present (with dates):

2. List all volunteer (medical or other) work experience (with dates):

3. List all research experiences (with dates):

4. List and describe your hobbies and recreational interests:

5. Indicate your extracurricular activities and the extent of your involvement (office held etc) while enrolled as a student:

6. List the names and relationships of relatives in the osteopathic profession:

7. How did you become interested in osteopathic medicine? (choose one) osteopathic physician, pre-health advisor, nycom admissions office, or other

8. In the space below (not a big space at all), discuss your reasons for applying to NYCOM.

9. They ask you to list where you requested recommendations from etc.

Finally a signature and photograph are requested (2 x 2)

I hope this helps anyone in need. It sucks you didn't have a secondary application with your interview invite. Goodluck

What day is your i nterview? I think I have a copy. (Or I at least have a xerox of the one I filled out). I can fax it to you if you have a fax number.
