I didn't indicate on my NYMC secondary that I would provide them with a fourth optional letter from a doctor that I shadowed. However, I'm planning on sending them that letter. Do you think they would ignore it because I didn't indicate on the secondary that I would send the fourth letter?

I don't think they will ignore it. However, they may review your application without it since they didn't realize it was coming. You may want to call NYMC and add it.
